Medical Photography Consent
I consent to medical photographs being taken of myself, my child, or the person for whom I am the legal guardian, as part of the medical assessment. This may include images of moles, skin conditions, other medical conditions, surgical procedures, or total body photography for mole mapping purposes.
These photographs will remain confidential and will not be shared or transferred, except in the case of total body mole mapping images. These images may only be transferred to another clinic that uses the FotoFinder Bodystudio system, and only with the patient’s explicit written consent. This ensures privacy and compliance with data protection regulations. Fees may apply.
I understand that I may nominate a General Practitioner (GP) to receive a medical report regarding my care. Medical reports will only be provided once treatment has been fully completed, including any required procedures.

Deposits & Fees – Procedure Appointments
- Deposits or out-of-pocket fees for surgical, cosmetic, and laser procedures must be paid in advance at the time of booking.All payments are non-refundable.

Rescheduling & Cancellations:
- If you reschedule or cancel at least 7 days prior to your appointment, your deposit will be credited to your account for future use.
- If you reschedule less than 7 days prior to your appointment or fail to attend, the deposit will be forfeited.
- If you reschedule or cancel twice or more with more than 7 days’ notice, the deposit will be forfeited, and full payment will be required upfront to secure a new booking.

- Deposits or out-of-pocket fees for certain consultations (including cosmetic consultations, skin checks/spot checks, and dermatology consultations) must be paid in advance at the time of booking.
- All payments are non-refundable.

- If you reschedule or cancel at least 48 hours prior to your appointment, your deposit will be credited to your account for future use.
- If you reschedule, cancel, or fail to attend less than 48 hours prior, the payment will be forfeited.
- If you reschedule or cancel twice or more with more than 48 hours’ notice, the deposit will be forfeited, and full payment will be required upfront to secure a new booking.
Non-Prepaid Consultation Appointments
(Full Body Skin Checks and Full Body Mole Mapping)
Rescheduling & Cancellations:
- If you reschedule or cancel at least 48 hours prior to your appointment, you may do so up to two times without charge.
- After two reschedules/cancellations, full upfront payment will be required to book any future appointment.
- If you reschedule or cancel less than 48 hours prior, you will be required to pay upfront for your next appointment.
- This prepaid amount will be forfeited if you fail to attend your next appointment.
